Additional practice documents
Remote Session Addendum — conditional draft
Client, named practitioner, date, platform, client location/jurisdiction, session purpose, length, fee, and final policy version:
Before confirming a remote appointment, review whether the practitioner may provide the proposed service where the client is located. Identify an appropriate private communication platform and explain its privacy limitations. A quiet private setting, stable connection, and ability to stop are needed. No session should occur while driving or operating equipment.
Agree how to pause, reconnect after a dropped call, and handle interrupted-session charges. The practitioner should not assume continued consent when the connection is lost. Identify any appropriate emergency plan for the particular service without treating the session as emergency care.
For any distance/proxy method, describe exactly what “proxy” means, who participates, what information or tools are involved, and the client's specific agreement. Do not assume permission to work on absent family members or third parties.
Private Session Recording Agreement — conditional draft
A recording is optional and separate from publicity permission. Service access must not depend on agreeing to marketing use.
Client and practitioner, date, platform/device, audio or video, purpose, and who will receive a copy:
Document where the recording is stored, access, delivery method, retention/deletion date, whether recording can be paused, and what happens if permission is withdrawn before or during capture. These details must be completed before recording starts.
Do not post, share with guest practitioners, use in training or promotion, or upload to an AI/transcription service without a separate specific explanation and agreement. A participant receiving a personal copy should protect it and avoid disclosing another person's information without permission. All participants must be informed and agree before recording.
Sound Bath & Group Participation Addendum — conditional draft
Event, named instructor, location, date, duration, price, and cancellation terms:
Explain the actual activities, sound levels, movement demands, touch policy, physical setting, and available modifications. Ask participants to raise relevant participation concerns privately, without requiring them to tell the group a health or trauma history. Participants may remain seated, modify movement, decline an activity, request a quieter position, or leave as appropriate.
Proposed group guidelines: respect personal space; avoid photographing or recording other participants; do not share others' personal stories; follow agreed room and equipment rules; do not touch instruments without permission. A group confidentiality request cannot guarantee that every attendee will keep information private.
Financial treatment of leaving early must be explained before payment. Any risk acknowledgment or liability release requires service-specific Texas review; this template is not a release of negligence or statutory rights.
Minor Participation & Guardian Permission — conditional draft
Adult participation is the planning default until a minor-service policy is separately approved. This template does not launch services for minors or resolve legal guardianship, confidential communications, safeguarding, or mandatory-reporting obligations.
Before use, set minimum age, appropriate services, guardian-presence rules, instructor training, emergency process, jurisdictional requirements, and insurance coverage.
Minor's name/age; guardian's name/contact; relationship and authority; named practitioner and proposed service:
Explain the activity to the child in age-appropriate language. Obtain guardian permission where legally required and the child's willing participation. A guardian's agreement does not override the child's refusal, distress, or request to stop. No broad permission for hypnosis, touch, recording, or all blended methods is assumed.
Specific activities, alternatives, supervision, boundaries, emergency plan:
Child's assent recorded in age-appropriate words by the practitioner:

Accessibility & Participation Statement — draft
BeYoutiful Harmony aims to make information readable and participation welcoming. Discuss seating, movement, sound, communication, or access preferences before arranging a session. The practice will explain available options and any setting limitations so you can decide whether the experience fits your needs.
The site uses responsive layouts, text alternatives for images, visible focus indicators, and standard disclosure controls. These design choices are not a certification of legal compliance or a claim that every feature has been independently audited.
A monitored accessibility contact and practical location details must be added before booking opens. Ask for assistance through that channel if information is difficult to access. Do not promise a particular accommodation before confirming it can be provided.
Privacy Request & Record Handling — draft templates
Before use, select the record system, retention periods, responsible person, privacy-request contact, response procedure, and applicable legal obligations.
Client request worksheet
Name and safe response contact; request date; request type (access, correction, deletion, sharing question, marketing opt-out, or other):
Describe the request without attaching unnecessary sensitive documents. The practice should verify identity proportionately and explain any limits, retention duties, or rights that apply. This template does not set a legally binding response deadline or promise rights beyond those applicable.
Internal record and incident log
Record only necessary operational facts. Restrict access. If a concern involves unauthorized disclosure, preserve appropriate evidence and follow applicable breach-response requirements with professional advice.
